Request a Sick Note
If you have been off work sick for 4 or more days in a row (including non-working days) your employer may ask for a proof of your illness. Your GP can provide a sick note.
Have you been off work sick for less than or more than 7 days in a row?
Request a sick note for less than 7 days
You do not require a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.
If you require a private sick note, you will be charged a fee.
